The possibility of DCDC to be punctured

Hello~ Smiley

Recently I am involved in an electronic project. However,I’m just a electronic enthusiast and my professional knowledge is limited.So here I am.
I designed a product by using MC34063 to be a DCDC of depressurization 24-->>5V. ( This is the MC34063's datasheet: http://www.kynix.com/uploadfiles/pdf2286/MC34063ABD-TR.pdf)The mass production is prepared to do and up to now It has been make over 200pcs. This chip didn’t has any phenomenon of puncturing.
Well, I have do some hardware before and I also saw some ACDC were punctured. So here I would like to do a survey that anyone witnessed the DCDC puncturing. Besides, will it be punctured if I manufacture more products ? Has anybody met such situation before ?
